Course Goals
Attendees will learn skills and tools necessary to apply State Licensure Care Coordination rules in daily operations.
Course Content
This Certificate Course includes the following curriculum, with interactive discussions, presentations and case studies. The curriculum includes:
Review of licensure standards for Care Coordination
Common survey focus areas and frequently cited deficiencies
Challenges to monitor in Care Coordination efforts
Understand Care Coordination barriers, measures of effectiveness, evaluation of your policy and procedure for today’s care settings
Competency evaluation
After this course, you will be able to:
Discuss the minimum standards for care coordination
List at least two current trends in citing deficiencies
Identify weakness in your agency relative to common challenges in Care Coordination program
Demonstrate an understanding of Care Coordination and patient/client/family communications
Course Materials
Registrants receive two workbooks for the online training: one with slides and the other with sample forms and self-exercises.
Continuing Education Units
Each registrant receives Continuing Education Units for up to two (2) clock hours of education at the completion of the course, course evaluation and competency exam with 80% score or better.
